Laser vein treatment uses a focused wavelength of light, absorbed by hemoglobin inside the vein, to heat and close the vessel from the outside of the skin. The body then reabsorbs the closed vessel over several weeks. No needle enters the vein and no sclerosing agent is injected.

Laser vein treatment tends to be a good fit for people who want a physician-led, non-invasive option for small, surface vessels, and who want an honest read on whether their veins are a laser problem or a vascular problem. You may be a candidate if you:

What we do not treat with laser

Being honest here saves you time, money, and frustration.

Laser vein treatment on the Harmony XL Pro is not the right tool for larger varicose veins, the ropey, bulging veins that stand up above the skin; symptomatic venous disease, including significant leg heaviness, chronic swelling, skin discoloration around the ankle, venous stasis changes, or venous ulcers; or suspected chronic venous insufficiency where the underlying problem is deeper venous reflux, not just a surface vessel.

Those conditions need a vascular workup, typically starting with a venous duplex ultrasound, and are treated with different technologies such as endovenous ablation, sclerotherapy, phlebectomy, or vein ligation performed by a vein or vascular specialist.

If your evaluation with Dr. Shemiranei shows any of those signs, he will tell you and refer you to a vascular specialist rather than treat you with laser. That referral is the right medicine, not a lost sale.

The technology: Alma Harmony XL Pro long-pulsed Nd:YAG

The Alma Harmony XL Pro is a multi-application aesthetic laser platform from Alma, cleared by the FDA under 510(k) K072564 [1]. It supports multiple interchangeable applicators, including a long-pulsed 1064 nm Nd:YAG applicator used for deeper vascular lesions and a Dye-VL vascular applicator for surface redness and small facial vessels [2].

Long-pulsed 1064 nm Nd:YAG is one of the standard, widely studied wavelengths for treating superficial cutaneous vascular lesions [4][5]. The 1064 nm wavelength penetrates deeper than shorter wavelengths, which is why it is well suited to blue and purple leg vessels, while shorter wavelengths and pulsed-light applicators are better matched to fine red facial vessels [3][6].

Dr. Shemiranei selects the applicator, wavelength, spot size, and pulse settings based on your skin tone and the vessel being treated, at the time of the appointment. Fixed protocols are not published online because settings are individualized.

Effective for small, superficial spider veins, facial telangiectasias, and rosacea-related redness in appropriate candidates [3][4][5][6][8]. Physician-supervised on an FDA-cleared platform.

Vascular workup first.

Laser vein treatment is not a substitute for a vascular workup when veins signal deeper venous disease.

Recurrence is possible.

New vessels can form and existing vessels can partially return over time.

Response varies.

Response varies by vessel size, depth, color, skin tone, and location. Some vessels need multiple sessions.

Temporary effects.

Temporary bruising, redness, swelling, and darkening over treated vessels are common.

Skin-tone consideration.

Post-inflammatory hyperpigmentation is a possible side effect, particularly in darker skin tones [9], which is one reason we use conservative settings and a test spot.

Referral when needed.

We do not perform sclerotherapy or endovenous ablation. If your veins are better treated by those, you will be referred to a vascular specialist.

Frequently asked questions

How many sessions will I need?

Most patients need a series of sessions spaced roughly 4 to 8 weeks apart. Small facial vessels sometimes clear in 1 to 3 sessions. Leg telangiectasias typically need more, often 2 to 4, and larger or resistant vessels may need additional passes [4][5][7]. Dr. Shemiranei will estimate a range after the evaluation, and reassess after each session.

Does laser vein treatment hurt?

Most patients describe the sensation as a quick snap, similar to a rubber band flick, cooled by the applicator’s built-in cooling. Comfort varies by area and vessel. We use a test spot at the first visit and adjust from there.

Is there downtime?

Usually no true downtime. Temporary redness, mild swelling, small bruising, or a slight tan-like darkening over the treated vessels is common and typically fades over days to a few weeks. You can go back to work the same day.

How is laser different from sclerotherapy?

Sclerotherapy is a needle-based treatment where a solution is injected into the vein to close it from the inside. It is considered the standard of care for many leg spider veins and small varicose veins and is often the first-line choice for vessels above about 1 mm [7][10]. Laser is needle-free and useful for smaller, superficial vessels, for vessels that are too small to inject safely, and for facial vessels where injection is not appropriate. A randomized head-to-head study of leg telangiectasias found long-pulsed Nd:YAG and polidocanol sclerotherapy both effective, with Nd:YAG particularly useful for vessels under 1 mm [7][10]. At APMUC we do not offer sclerotherapy on-site, so if that is a better fit for your veins, you will be referred out.

What about sun avoidance and compression?

Sun exposure on treated areas should be minimized before and after each session, and sunscreen should be used on treated skin. Some patients wear compression stockings after leg treatments to support the closing vessels. Dr. Shemiranei will give you the specific guidance for your case.

How long until I see results?

The treated vessels typically fade gradually over several weeks as the body clears them. Full response is usually judged 6 to 8 weeks after each session, which is why sessions are spaced accordingly.

Are results permanent?

Treated vessels that clear are typically gone for good, but new vessels can form over time due to genetics, hormones, sun exposure, standing occupations, pregnancy, or underlying venous factors. Recurrence is possible, and we do not promise otherwise. Some patients come back for maintenance sessions in future years.

Is laser vein treatment safe for darker skin tones?

Long-pulsed 1064 nm Nd:YAG is generally considered one of the safer laser wavelengths across the Fitzpatrick scale, including in darker skin, because 1064 nm is less absorbed by melanin than shorter wavelengths [9]. Post-inflammatory hyperpigmentation (temporary darkening) is still a possible side effect, particularly in darker skin, which is one of the reasons a physician-supervised test spot matters.

Who is not a candidate for laser vein treatment?

Patients with active infection or wound at the treatment area, patients with photosensitizing medications or conditions, and patients whose veins point to underlying venous disease that needs a vascular workup are not candidates for laser alone. Pregnancy and certain skin conditions are also considered case by case. Dr. Shemiranei screens for all of these at the evaluation.
