Dr. Saied Shemiranei, medical director and founder of APMUC, has cared for older adults in Orlando, Florida throughout his career. He believes geriatric primary care requires something most large systems don’t offer: time. Time to review ten medications. Time to ask about cognition. Our practice is not a revolving door. Older patients at APMUC see the same physician at every visit, and that physician knows their history. We accept Medicare, and we welcome families. If your parent needs more than an annual checkup, we’re equipped to manage it.

Do you accept Medicare?

Yes. Medicare Part B covers the annual wellness visit, most chronic disease management visits, preventive vaccinations, and many lab orders. We will clarify coverage for your specific plan before billing.

What is an Annual Wellness Visit (AWV)?

The Medicare Annual Wellness Visit is a dedicated preventive visit – not a sick visit – where we review your health history, update your preventive plan, screen for cognitive changes and fall risk, and coordinate your upcoming year of care.

Is the polypharmacy review really necessary?

For patients on five or more medications, yes. Drug interactions and unnecessary medications are a leading cause of falls, hospitalizations, and cognitive decline in older adults. We take it seriously.

Do you do cognitive screening?

Yes. We use validated brief assessments at every annual visit. Early identification of cognitive changes allows for earlier planning, family conversations, and specialist referral when appropriate.

Can family members attend the appointment?

Yes, with the patient’s permission. We can also provide a written care summary for family members who can’t be present in person.

What vaccinations do you offer?

We administer flu, shingles (Shingrix), pneumonia (Prevnar 23 and Prevnar 20), and COVID-19 vaccines in-office. We check your immunization record and recommend what’s overdue.

Do you coordinate home health services?

Yes. If an older patient needs home health aides, physical therapy at home, or durable medical equipment, we write the orders and coordinate with home health agencies.

What if my parent has dementia?

We manage early and moderate dementia as part of primary care, including medication review, family education, and safety planning. For advanced dementia or behavioral complications, we coordinate with neurologists and memory care specialists.

Do you see patients who only speak Spanish or Farsi?

Yes. Our team speaks English, Spanish, Farsi, French, and Portuguese. Language should never be a barrier to good senior care.
